All Care Home Care, Inc., located in Hazleton since 1986, is a locally owned and operated home health agency. All Care is dedicated to providing safe and high quality home health care, treatment and services to sick, frail or disables individuals. The agency is Medicare/Medicaid certified and is accredited by Joint Commission Accreditation on Healthcare Organizations.

All Care Home Care provides prompt, efficient, highly skilled care 24 hours a day, 7 days per week. Our service areas include: Luzerne, Carbon, Schuylkill and Columbia Counties. The professional staff consists of full-time, part-time, per diem Registered Nurses and Licensed Practical Nurses. The RN nursing staff is PICC line and IV certified.
All Care Home Care's Rehabilitation Services include a staff of licensed physical therapists who provide services within 24 hours of referral. Occupational Therapists along with our Speech Therapists provide quality rehabilitative services in the patient's home. Therapists assist patients to become as independent as they possibly can and to cope with their physical limitations.
Social services and short-term counseling are provided by our Licensed Medical Social Worker. A registered Dietitian is available for nutrition and diet counseling. Bathing and personal care services are provided by our caring staff of Certified Home Health Aides and Licensed Practical nurses. All Care Home Care in close association with the patient's physician and health plan, offers a multidisciplinary approach in meeting the patient's physical and psychosocial needs
Center for Medicare Services established the Home Health Compare (HHC) website on Medicare.gov as a key tool for consumers to use when choosing a home health care provider.
The Home Health Agencies are rated on 9 measures. They include three process measures: the timely initiation of care; education provided to the patient or their caregiver about their medications; and the administration of flu shots. Also included are six outcome measures for improvements in ambulation, bed transfers, bathing, pain, shortness of breath and acute-care hospitalization.
