Mission Statement

All Care Home Cares primary mission is to provide comprehensive, safe and high quality home health care treatment and services to the sick, frail or disabled individuals of Luzerne, Carbon, and Columbia Counties. Home health care is the professional and paraprofessionals provide care, treatment and services to the patient in their home setting under the direction of the patient's physician.

These services are provided for the purpose of improving the health status, maintaining health status to fulfill the patient's needs and goals. These needs and goals will be planned and implemented by this agency and authorized by the patients's physician.

As an agency, we feel home health care, treatment and service will ultimately improve the quality of life for the individuals who can benefit from our service. By recieving these services, a goal is to reduce the over-use of long-term care facilities and acute care facilities, consequently enabling more effective and efficient use of the limited number of hospital beds available in this area.

We furter beliece that as part of the community, we have a responsibility to promote and support health prevention activities which will enhance the quality of life and improve the standard of health care for individual of the community.
